Provides nursing and related services, which enable each resident to attain or maintain their highest practical level of physical, mental and psychosocial well being. Performs duties and responsibilities in a manner consistent with our mission and values.
1. Assists with Human Resource Management. 20%
a. Performance Monitoring: assists with conducting performance evaluations and Counseling for all staff; assists team members in setting short and long-term goals; assists with the
disciplinary process of individual staff.
b. Recruitment/Hiring: assists/conducts interviews with potential new hires.
c. Role Model for FACES culture, ensuring employees have adequate training and display FACES culture expectations.
d. Ensured employee attendance is reviewed.
e. Utilize appropriate process for progressive discipline by:
1. Disciplining individual staff as delineated in hospital policy and nursing standards at the same time demonstrating knowledge of labor laws applicable to hospital personnel.
2. Terminating employees after review and approval of Director, Administrator, and review by Human resources.
f. Ensures staff has knowledge of locations and use of policy/procedures.
g. Responsible for employee payroll.
h. Communicates with employee’s pertinent information in coordination with Human Resources.
i. Tracks annual PPD to ensure all employees are current.
2. Coordinates unit functions and responsibilities relating to management of unit. 10%
a. Staff Scheduling: ensuring 24-hour coverage appropriate to patient needs, assuring that the quality and quantity of staffing ratios are equitable.
b. Provides a proper balance between administrative responsibilities and clinical involvement
c. Demonstrates the ability to adapt to changing priorities on the unit.
d. Considers the needs of other units, as well as his or her own, and includes this concern in decisions for staffing and assignment change.
e. Delegates Duties based on competence, using personnel in an efficient and cost-effective manner.
f. Assigns staff responsibilities to enhance knowledge and develop new skills.
3. Ensuring compliance with Medicare/CMS/DHSS long term regulatory compliance in their area of responsibility.10%
4. Establishes and maintains Rapport and working relationships with Nursing staff, patients and other members of the healthcare team. 20%
a. Addresses the healthcare team, residents, families, and visitors with human dignity and respect.
b. Develops working relationships that are characterized by mutual support, open communication, trust, and respect, which promote positive attitudes and good morale on the unit. Provides
frequent informal feedback and comprehensive annual performance appraisals.
c. Communicates problems to the appropriate person and/or Director.

e. Offers suggestions for growth or change to others in a private constructive manner.
f. Projects positive image of self and facility when dealing with staff, physicians, other departments, residents, family members, and visitors.

h. Promotes open communication within the unit, with other units and departments, as well as other members of the healthcare team.
i. Provides a means of communication to all staff members.

m. Maintains ongoing communication with residents and/or family regarding current status.

5. Assists with skin care program. 10%
6. Supports organizational goals for patients and employee satisfaction by performing rounds, monitoring and making sure call lights are being answered on a timely manner, makes sure physicians are being called on a timely manner, and staff is addressing residents pain in a timely manner. 10%
7. Promotes a safe environment by utilizing safety practices as established for unit and facility. 5%
a. Assist with Fall prevention and management program.
b. Ensures resident safety related to proper lift mode and use of gait belts.
c. Ensures proper use of side rails.
8. Assists with Financial management 5%
a. Responsible for Employees payroll.
b. Ensures unit staffing to acuity.
c. Supervises and educates staff on proper utilization of supplies.
d. Assist in implementing cost saving measures.
9. Participates in Quality Improvement activities. 5%
10. Completes audits of records as necessary to assess compliance with policy/protocols. 1%
11. Ensures timely completion of monthly weights and vital signs, and change over of physician orders. 1%
12. Follows up with physicians to communicate Pharmacy Drug Regimen Review. 0.5%
13. Assist with MDS process to update and implement plans of care. 0.5%
14. Attends Leadership classes. 0.5%
15. Ensures all new hire’s receive proper orientation to perform their job. 0.5%

Employee may be required to perform simple grasping and pushing/pulling.

Nursing Diploma or Associates in Nursing required.
