Role Description
This is a full-time remote role with travel to our branches in Texas, New Mexico, Idaho and Colorado for a Quality Assurance Nurse - Hospice. The Quality Assurance Nurse will be responsible for ensuring compliance with regulatory requirements and adherence to quality standards in the delivery of hospice services. The Quality Assurance Nurse will conduct audits, identify areas for improvement, and develop and implement quality improvement initiatives. They will also provide guidance and education to leaders and staff to ensure adherence to policies and procedures and compliance with industry standards.
Functions & Responsibilities
- Responsible for conducting an ongoing comprehensive and integrated Agency assessment of the quality and appropriateness of the care provided, including services provided under arrangement.
- Aggregates and trends data collected to correct identified problems in collaboration with the Vice President of Quality & Compliance.
- Monitors all service providers for effective interchange and coordination of care in providing care.
- Prepares quarterly Quality Improvement report and collaborates with agency leaders to ensure ongoing quality improvement.
- Participates and assist agency leaders in preparation and ongoing monitoring of PIP and QA Actions Plans.
- Prepares annual Quality Improvement report as part of annual agency evaluation.
- Conducts annual Safety Surveillance Evaluation.
- Participates in agency quarterly QAPI meetings.
- Utilizes clinical knowledge to coach and mentor site clinical leaders to drive clinical excellence and addresses areas of improvement on clinical documentation and patient care.
- Partners with agencies to determine individual needs and coordinates site visits to meet the identified needs of each program.
- Partners with the Vice President of Quality and Compliance to ensure training and education are provided at the agency level by collaborating with Agency Leadership.
- Provides support via phone, email, or visits to Clinical Leaders and colleagues concerning issues of patient care, appropriateness, and compliance.
- Provides support via phone or visit as needed for state/federal/accrediting surveys.
- Maintains expertise in clinical knowledge of CMS HIS Guidelines.
Qualifications
- Valid and unrestricted Registered Nurse (RN) license
- Minimum of 5 years of experience in hospice nursing
- 3-5 years supervisory experience
- Associate or bachelor's degree in nursing
- Strong knowledge of hospice regulations, including Medicare Conditions of Participation
- Knowledge of Nurse Practice Act and acceptable standards of Nursing Practice.
- Must have strong working knowledge of QAPI principles, techniques, and implementation processes to drive regulatory and policy standards.
- Proficient in Microsoft Office suite, Electronic Medical Records systems, Medicare online portals &/or other related electronic sources and tools.
- Experience conducting quality assurance audits and developing quality improvement plans
- Excellent communication and interpersonal skills
- Detail-oriented with strong analytical and problem-solving abilities
- Ability to work independently and collaboratively in a remote setting
- Hospice and Palliative Care Certified Nurse (HPCCN) certification is a plus
