Title: CityHouse Community Care Program Case Manager/Family Advocate
Status: Full-Time
Hours: 40 hours/week
Reports to: Program Director
SUMMARY OF ROLE:
This position primarily creates and implements an individualized case plan for each CityHouse resident. The Family Advocate/Case Manager supports and advocates for families to achieve independence and self-sufficiency by helping them identify and address issues that impact their economic, physical, and emotional well-being. Facilitate goal setting, connecting residents to community resources, case management, and accountability for the women and their children. The Case Manager/Family Advocate ensures that CityHouse identifies, connects, and works well with local partnering organizations and provides appropriate resources to each family based on their unique needs.
ORGANIZATIONAL OBJECTIVE:
CityHouse Inc. provides hope and renewal to single mothers and their children who are experiencing homelessness through a long-term transitional housing program. CityHouse staff and board members are rooted in a solid Christian foundation and share a common belief in the values and teachings of the Gospel.
PRIMARY RESPONSIBILITIES:
Case Management:
- Plan, schedule, and uphold supportive case management sessions
- Coordinate volunteers for each family’s needs
- Provide linkage to community resources
- Research and provide updates for a website portal library with community resources
- Submit referrals for services to the appropriate organizations
- Build a mentorship relationship with the mothers and their children
- Participate in the first and second interviews for the family selection process
- Supervise and give tasks to BSW and/or MSW interns
- Provide support to CCP (Community Care Program) families and their children
- Create a community amongst the CCP families
- Help coordinate access to the pantry, clothing closet, or laundry room as needed
- Facilitate monthly meetings to provide preventative and non-reactive care
- Provide monthly home visits for the CCP families
- Attend all CityHouse staff meetings and devotionals on Tuesdays
Vocational/Educational:
- Work with mothers to map their finances/budget and build long-term, realistic, achievable goals.
- Assess career and education goals and incorporate them into individuals' treatment plan
- Utilize career assessments to guide mothers to future career opportunities.
- Develop educational plans including timelines to apply for colleges, certification programs, and financial aid.
- Assist mothers with filling out and completing educational applications and providing support for deadlines.
- Research requirements and locate GED-focused programs, classes, and tutors.
- Work with mothers on workplace job skills, work ethics, and communication skills to assist them in reaching career goals.
- Research, find, and build relationships with community college and career center connections for better work opportunities and build a bridge to the women in the program.
- Identify and hold classes about financial and career goals in our learning center
Children’s Case Management:
- Identify strengths, limitations, risk factors, and needs, and refer and coordinate the provision of concrete services to the child in conjunction with the Program Director.
- Provide a written assessment of each child, then with the team, promptly develop and implement a case management plan.
- Provide support and supervision to children on an as-needed basis
Documentation:
- Prepare progress updates for staffing on a bi-weekly basis.
- Complete bi-weekly documentation utilizing case management software, Apricot, for each CCP family. This includes but is not limited to, alumni narrative notes, alumni self-sufficiency, and other assessments on an as-needed basis.
- Monitor the progress of services being offered and update the team during weekly staffing meetings on needs for completion.
- Weekly monitoring and reviewing case plans with team members and properly documenting any updates in Apricot.
- Complete all non-clinical intake paperwork for case management services. This includes gathering historical and relevant background information and records (medical, school, legal, etc.)
- Attend and participate in weekly staffing review meetings.
- Refer the child, as appropriate, for medical care and evaluations, including developmental and psychiatric, educational, and enrichment activities, etc.
- Work with staff and interns to create a monthly calendar of community events focused on activities (ex. SRC Kids, Awana, YMCA, Boys & Girls Club, etc.)
- Work with interns to organize group activities on the property (ex. painting classes, sports activities, seasonal parties, etc.)
- Assist mothers in identifying, scheduling, and planning enrichment activities for their children.
- Encourage mothers to be actively involved with their children’s school teachers and attend IEP or ESE meetings with parents when/if necessary.
- Prepare progress updates for the Program Director and team during weekly staff meetings.
- Complete weekly documentation in case management software Apricot for each resident and family.
Qualifications:
- Commitment to confidentiality
- Complete a criminal background screening and drug screening.
- Commitment to the Ethical Standards and Principles set by the National Association of Social Workers
- Minimum 2 years of experience as a Case Manager or similar field; working with women and children preferred
- Familiar with case management software and utilization of documentation and reports
- Superior listening skills, strong oral and written communication skills
- Ability to work with diverse clients and a network of volunteers, supporters, and agencies in a friendly and professional manner
- Knowledge of and ability to connect with Broward County community resources
- Strong interpersonal skills and team-focused
- Demonstrates knowledge of Trauma Informed Care specific interventions
