- Monday to Friday, 8:00 AM - 4:30 PM EST
- Monday to Friday, 11:00 AM - 7:30 PM EST
- After six months, choose from flexible schedules:
- Four 8-hour days
- Four 9-hour days
- Four 10-hour days

**Responsibilities of a Chronic Care Manager**
- Conduct regular patient check-ins to discuss their status and ensure adherence to medical appointments.
- Build rapport and trust with patients and their families.
- Collaborate with the patient's care team, ensuring effective communication with providers and office staff.
- Use your nursing expertise to:
- Assist patients in prioritizing their health and understanding their medical conditions.
- Advocate for patient needs and address medical concerns.
- Employ technology to coordinate care and set health goals.
- Utilize Electronic Medical Records (EMR) systems for:
- Reviewing recent office visits.
- Collaborating with the care team.
- Accurately documenting all patient interactions.
**Qualifications**
We are looking for compassionate individuals with the following qualifications:
- **Licensing**: Must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in any U.S. state (licensing fees for out-of-state patients will be covered).
- **Experience**: Minimum of four years in nursing care for chronically ill patients; home health or primary care experience is an advantage.
- **Technical Skills**: Proficiency in EMR systems and a willingness to learn and adapt to new technology.
- **Time Management**: Strong ability to manage time effectively, particularly in a remote setting.
- **Communication**: Excellent active listening and clear verbal/written communication skills.
