### Role Overview
In this remote position, you will play a crucial role as part of a clinical team, focusing on comprehensive case management that strengthens the bond between patients and their primary care providers. Your role involves assisting patients to reach their highest level of wellness, particularly those with complex medical conditions, socio-economic challenges, or mental health co-morbidities.
### Key Responsibilities
- Conduct thorough assessments of patients' healthcare and psychosocial needs.
- Refine patient navigator interviews for accuracy and clarity.
- Develop and maintain individualized care plans in compliance with internal protocols and accreditation standards.
- Implement evidence-based clinical interventions aligned with care plan goals.
- Collaborate with patient navigators to manage non-clinical aspects of care, such as appointment scheduling and addressing social determinants of health.
- Monitor patient progress and proactively adjust care plans as necessary.
- Partner with primary care providers to create patient-specific escalation plans for managing chronic conditions.
- Educate patients regarding their medical conditions and treatment options.
- Advocate for patients to help them access necessary services and resources.
- Ensure accurate and up-to-date documentation in the electronic health record.
- Adhere to established quality metrics and clinical protocols.
### Qualifications
- Current Registered Nurse (RN) license in good standing.
- Minimum of 3 years of RN Case Management experience in an outpatient setting.
- CCM certification preferred, with a requirement to obtain it within one year of employment if not already certified.
- Supervisory experience is advantageous.
- Strong written and verbal communication skills for empathetic patient interaction.
- Proficient computer skills in EMR and case management systems.
- Ability to meet NCQA and HEDIS accreditation and quality standards.
- Bilingual in English and Spanish is a plus.

### Compensation
The annual base pay for this role ranges from $80,000 to $85,000, paid bi-weekly. Final compensation will be determined based on skills, experience, education, and location.
