Your Role
In this position, you will be instrumental in enhancing the connection between patients and their primary care teams. You will focus on improving patient health, navigating care processes, and facilitating effective communication with medical practices.
Key Responsibilities
- Remote Work & Collaboration: Collaborate remotely with patients, medical practice staff, and our Case Management team to manage a targeted patient population.
- Patient Engagement: Assist patients in achieving improved health outcomes, addressing inquiries, connecting them with medical practices, and supporting care plan implementation.
- Documentation: Accurately document patients’ medical histories and issues in the electronic health record (EHR).
- Referral Process: Understand and navigate the referral and prior authorization processes.
- Relationship Building: Establish and maintain professional relationships with assigned medical practices and within our organization.
- Performance Metrics: Achieve assigned Key Performance Indicators to promote positive patient outcomes and decrease medical expenses.
- Communication: Employ various communication methods (calls, texts, portals, emails) to reach and engage high-risk patients.
- Issue Escalation: Escalate medical issues to the Health Guide Registered Nurse, Primary Care Physician, or leadership as necessary.
- Travel: Occasionally visit supported primary care physician offices.
- Security: Comply with security policies to safeguard all Protected Health Information (PHI) and intellectual properties.
Qualifications
- Certification: Certified or Registered Medical Assistant.
- Bilingual Skills: Fluent in English and Spanish.
- Patient Coordination: Passionate about coordinating care for patients with complex medical and social needs.
- Empathy & Skills: Strong empathy, motivational interviewing skills, and effective patient engagement abilities.
- Documentation: Excellent conversational skills and experience documenting patient interactions in real-time.
- Medical Knowledge: Familiarity with medical terminology, primary care functions, and social determinants of health.
- Technical Skills: Proficiency with EHRs, care management analytics, and various software programs.
- Attention to Detail: Skillful in managing competing priorities while maintaining accuracy.
- Remote Work Experience: Proven success in a remote work environment.

Compensation
Hourly rate between \(20 to \)23, influenced by experience, education, and location.
