This fully remote position involves occasional travel to client locations (up to 10%). We welcome applications from local candidates who live within a 90-minute drive of Decatur, GA.
Responsibilities
- Collaborate with patients' medical practice staff and the Case Management team to manage a specific population of patients.
- Work independently while also being an integral team member, extending the medical practice’s capabilities.
- Assist patients in achieving better health outcomes by addressing their questions and concerns, connecting them with medical practices, and helping implement care plans.
- Accurately document patients' medical histories and issues in the electronic health record (EHR).
- Understand the referral and prior authorization process.
- Build and maintain trusting professional relationships with assigned medical practices and within our organization.
- Implement and achieve Key Performance Indicators (KPIs) to promote positive patient health outcomes and reduce medical spending.
- Engage high-risk patients using effective communication methods (calls, texts, portals, emails).
- Escalate medical issues appropriately to the Health Guide Registered Nurse, primary care physicians, or leadership.
- Travel occasionally to supported primary care physician offices.
- Follow security policies and procedures to protect all Protected Health Information (PHI) and proprietary information.
Requirements
- Certification or registration as a Medical Assistant.
- Bilingual skills in English and Arabic are required.
- A passion for patient coordination and serving individuals with complex medical and social needs.
- Empathy and a willingness to develop skills in motivational interviewing, active listening, and behavioral change management.
- Exceptional communication skills and the ability to document patient interviews accurately and in real-time.
- Familiarity with medical terminology and the functions of a primary care practice.
- Understanding of social determinants of health.
- Certification as an Emergency Medical Technician (preferred).
- Proficiency in electronic health records (EHRs) and various software programs.
- Strong organizational skills to balance multiple priorities while remaining detail-oriented.
- Familiarity with EHR systems such as EPIC, Athena, and Cerner.
- Excellent written, visual, and verbal communication skills.
- Ability to track multiple tasks and ensure timely completion.
- Capability to work independently as part of a patient's overall care team under the supervision of licensed clinical professionals.

Compensation
The base pay for this role ranges from \(20 to \)23 per hour, paid bi-weekly. Final pay decisions will consider various factors, including skills, experience, education, location, and certifications.
