Job Description
We have a great opportunity for a highly engaged and motivated Perinatal Nurse Navigator! The ideal candidate will provide complex obstetric patients with specialized education, patient advocacy, follow‐up, support, and coordination of care throughout the duration of their pregnancy and after discharge. This navigator will build trust and professional relationships with Broward community Obstetricians and Perinatologists aligned with the health system to connect with high risk mothers in underserved communities early in their pregnancy and serve as a guide and resource for them through their care journey. The nurse navigator will be the primary touchpoint for the patient while coordinating care with each of her supporting medical teams—Obstetrics, Maternal Fetal, Neonatology and any relevant subspecialties. This role offers a flexible schedule and requires travel between Davie and Margate, FL.

Responsibilities
These are your responsibilities :
- Care coordination for at/high-risk women through and after their pregnancy, beginning with obstetrics, MFM, necessary pediatric subspecialists, inpatient admission through and after discharge
- Serve as resource for hospital & community OBs for high-risk moms that are aligned with hospital system
- Follow the mothers/babies for a full year after the baby is discharged to provide support during postpartum, including post-partum anxiety and depression
- Aid in the development and management of a hospital system-wide transport protocols
- Maintain clinically appropriate births inside health system by developing delivery plans
- Help the families understand the level of care they need and that their baby will need
- Encourage prenatal compliance and ease their anxieties before delivering at our hospital partner facilities
- Assist patient in compliance with appointments through organization and optimization of patient experience
- Develop, input, and maintain a longitudinal quality and tracking dashboard that incorporates relevant data and key performance measures as it relates to maternal and neonatal outcomes
- Other duties as assigned
Qualifications
To perform this job successfully, an individual must be able to perform each essential responsibility satisfactorily. The requirements listed below are representative of the knowledge, skills and/or abilities required.
Education/Experience
- Minimum Education Requirements- Completion of accredited Registered Nursing program, BSN preferred
- Minimum experience - 3 years in recent active RN practice
- Strong nursing background in an acute (hospital) setting in OB/NICU requiring coordination of complex care activities
- Leadership/education roles within the nursing field is a plus (Charge nurse, manager, advanced practice provider)
- Ability to create and maintain strong professional relationships
- Passion for maternal/infant care with a primary focus in OB/NICU
Knowledge & Skills
- Candidate must have knowledge of Microsoft Office Suite, specifically Excel and Microsoft Word
- Use and/or knowledge of EMR preferred
- SharePoint experience helpful, not required
Certificates And Licenses
- Current RN license in good standing
