Job Summary
We are seeking a Chronic Care and Remote Patient Monitoring Manager to lead our chronic care management (CCM) and remote patient monitoring (RPM) programs. In this fully remote role, you'll work closely with healthcare providers, develop patient care plans, ensure regulatory compliance, and use data to enhance patient outcomes. Key Responsibilities
- Educate patients and their families on CCM, RPM devices, and self-care techniques.
- Provide exceptional customer service through meaningful and engaging interactions over the phone.
- Leverage your healthcare expertise to manage a monthly patient roster, ensuring comprehensive care for each individual.
- Maintain consistent, compliant services while creating and updating personalized care plans.
- Aim for a patient engagement rate of 90% or higher by regularly reaching out to patients and keeping the healthcare team informed.
- Conduct monthly assessments, review medications, and coordinate with family members and caregivers.
- Collaborate with healthcare providers, including physicians and nurses, to develop and implement patient care plans.
- Monitor and analyze patient data from RPM devices to identify trends and make care recommendations.
- Ensure compliance with healthcare regulations, including HIPAA and CMS guidelines, for CCM and RPM programs.
- Train and supervise a team of care coordinators and medical staff involved in CCM and RPM programs.
- Develop and maintain policies and procedures for chronic care and RPM programs.
- Prepare regular reports on program performance, patient outcomes, and areas for improvement.
- Participate in the hiring process for additional Chronic Care Case Management team members.
- Stay updated on industry best practices, emerging technologies, and regulatory changes related to chronic care and RPM.
Qualifications
- RN, CCRN, CRNA, MA, CCMA, CMA, RMA, or other relevant medical certification.
- Minimum of 2 years of experience in chronic care management, remote patient monitoring, or a related healthcare field.
- Strong understanding of chronic disease management, healthcare regulations, and remote monitoring technologies.
- Ability to maintain confidentiality and adhere to HIPAA guidelines.
- Skilled in handling difficult and sensitive conversations.
- Comfortable multitasking, including typing while talking.
- Proficient in EHR platforms and MS Windows, Word, Excel, and Outlook.
- Excellent leadership and team management skills.
- Strong analytical skills with the ability to interpret data and develop actionable plans.
- Proficiency in healthcare management software and remote monitoring tools.
- Excellent communication and interpersonal skills.
- Fluency in an additional language is a plus.
- Ability to work collaboratively in a multidisciplinary team environment.
- Strong organizational skills and attention to detail.
