Your Expertise In Action
- Care Coordination: Facilitate seamless communication and collaboration among the healthcare team to ensure efficient and effective care delivery.
- Transitional Care Management: Guide patients through their healthcare journey, developing and implementing comprehensive discharge plans that address their individual needs and support a smooth transition home or to another care setting.
- Patient Assessment: Conduct thorough assessments to identify and address physical, emotional, and social needs that may impact the patient's care.

Qualifications - Education:
- Bachelor's Degree in Nursing or equivalent education/experience is required.
- A Master's Degree in Nursing or Healthcare related field is preferred.
- RNs with 20 years of experience may be exempt from the BSN requirement.
- Licensure:
- Active California Registered Nurse License upon hire.
- Experience:
- 2+ years in a healthcare-related field (Acute, Ambulatory, or Post-Acute).
- 2+ years in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Managers. TIP candidates must have experience in the same type of nursing unit where the CM position is available.
- Certifications:
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification is preferred upon hire.
