Position Overview
As an RN Case Manager, you will play a key role in building relationships with clinical teams, patients, and healthcare providers. Your mission will be to help patients reach their highest level of wellness, especially those with complex medical, socioeconomic, or mental health conditions. You will create and oversee care plans, ensuring that patients receive the best possible care while coordinating with patient navigators and care teams to achieve optimal health outcomes. This position offers the unique opportunity to work remotely, with minimal travel required.
What You'll Do
- Conduct comprehensive assessments of patients' healthcare and psychosocial needs.
- Collaborate with patient navigators to review patient interviews and ensure alignment with care plans.
- Develop and maintain individualized care plans that meet internal protocols and industry standards.
- Implement evidence-based clinical interventions to support patient health goals.
- Work with patient navigators to handle non-clinical aspects of care, such as scheduling appointments or arranging for transportation and other services.
- Provide ongoing support through patient outreach and data monitoring, making adjustments to care plans as needed.
- Collaborate with primary care providers (PCPs) to develop escalation plans for managing chronic conditions.
- Educate patients about their health conditions and available treatment options.
- Act as a patient advocate, assisting them in accessing necessary clinical services and resources.
- Follow up on service or resource referrals, ensuring that all documentation is up to date and in compliance with protocols.
- Utilize evidence-based guidelines and quality metrics to guide care decisions.
What You'll Need
- Active Registered Nurse (RN) license in good standing.
- 3+ years of case management experience in an outpatient setting.
- Certified Case Manager (CCM) certification is preferred or willingness to obtain it within a year.
- Prior supervisory experience is a plus.
- Strong communication skills with the ability to engage empathetically with patients and work collaboratively with interdisciplinary teams.
- Proficient in using electronic medical records (EMR) systems and case management software.
- Knowledge of accreditation and quality standards, including NCQA and HEDIS.
Preferred Qualifications
- Bilingual (English/Spanish) skills are a plus.

Compensation
The salary range for this role is \(80,000 to \)85,000 annually, paid bi-weekly. Compensation will be determined based on factors such as skills, experience, education, and certifications.
