Description
The Clinical Logistics Nurse (CLN) is a member of the Core Clinical Partners Clinical Operations Department, responsible for delivery of Core’s process designed to improve quality and efficiency in the care of hospitalized patients. The CLN collaborates closely with Medical Directors to engage and educate the physician team, optimize performance on defined key metrics, and ensure collaboration with the care team. This position does not involve providing direct patient care.
Success in this role will be demonstrated by tangible improvements in patient outcomes and care experiences, along with more efficient resource utilization.
Essential Duties
- Participate in the development of site-specific workflows to achieve program goals and performance targets, aligning with hospital and Core priorities.
- Partner with the Medical Director to implement Core’s processes and lead process optimization.
- Provide timely status updates of implementation, barrier resolution, and action plans.
- Understand, clearly articulate, and promote the adoption of hospital and Core clinical performance objectives with the physician/APP team.
- Engage and educate clinician team(s) related to clinical performance initiatives, goals, and measurement.
- Oversee and facilitate clinical performance activities, potentially across multiple service lines
- of patients eligible for specific patient-centric care initiatives
- Participate in daily workflows by collaborating with various care team members
- Advocating for patient goals in the care plan
- Enhance communication throughout the care team as the patient moves through their hospital stay.
- Facilitate coordination between the hospital medicine team and hospital departments (e.g., Nursing, Quality, Case management, Clinical Documentation, Pharmacy, and others) to ensure the delivery of workflows related to:
- Throughput, discharge facilitation, and transitions of care
- Readmission prevention
- Patient experience
- Timely physician response to documentation queries
- Collaborate with the onsite team to establish and communicate patients’ care goals.
- Coordinate patient and family education to ensure understanding of the disease process, next-level-of-care (NLOC) options, and community resources.
- Coordinate and facilitate follow up and goals of care planning for patients identified to be in the end-of-life stage of their disease process.
- Ensure a process for scheduling community provider follow-up post discharge for patients determined to be elevated risk for readmission.
- Participate in initiative-taking evaluation of program metrics and initiate additional investigation of drivers when targets met
- Collaborate with the Medical Director, DCO, DOO, and hospital leaders to share performance data with the physician team to drive continuous improvement.
- Continually evaluate processes, tools, and workflows to identify opportunities for improvement
- Participate in hospital committees and special projects as appropriate
- Orient new physicians to Core’s processes, quality, and patient experience initiatives
- Collaborate with Medical Director, DOO, and DCO to contribute to team meeting agendas
- Serve as a point of contact for hospital staff (e.g., issues related to clinical processes and triage for other potential issues related to hospital medicine team)
- Coordinate educational forums as needed
- Other duties as assigned
Requirements
Skills, Knowledge, Abilities:
- Current RN licensure in good standing, in the state applicable to the position
- Strong clinical knowledge and experience in the Acute Care Hospital setting (Critical Care, In-patient Medicine, Emergency Department, Quality, and/or Case Management experience is helpful)
- Ability to assess patient’s clinical status and readiness for discharge
- Ability to work both independently and collaboratively
- Excellent organizational skills with the ability to multi-task and prioritize workload in a challenging environment while maintaining mindfulness and accuracy
- Excellent written and oral communication skills, including strong presentation skills
- Client service oriented with the ability to interact and communicate with individuals at all levels (e.g., patients, physicians, hospital staff, hospital administration and other colleagues)
- Strong people skills to manage sensitive situations and confidential information. Position continually requires demonstrated poise, tact, diplomacy, and good judgment
- Proficiency with technology, including electronic medical systems and Microsoft Office Suite

Education
- RN required
- MSN preferred
Experience
- Minimum: 5 years nursing experience, with at least 2 years recently in an inpatient acute care or hospital setting
- Proficient with electronic medical systems
- 5 or more years inpatient nursing experience and 1-2 experience in a nursing leadership role, Case Management, Quality, Transitional Care, and/or post-acute services preferred
- Knowledge of process improvement methodology. Lean Six Sigma Green Belt (LSSGB) certification or equivalent, preferred
