Position Summary: The Workforce Connections Program Case Manager role is a unique opportunity to significantly impact our community. As a key member of our team, you will work in collaboration with the Program Director to provide direct support services to students, ensuring all interactions between United Way, program participants, and partners are positive and productive. This role requires a multi-tasker with excellent communication skills and an upbeat attitude. The Case Manager will hold high ethical and moral standards, partnering these values with organizational skills and excellence while building rapport with participants and their families. Candidates should be able to maintain high-quality, timely client documentation and records management utilization of electronic charting. The Case Manager will develop, implement, and monitor the Individual Service Plan, revising participant goals once met or progress as they occur. Knowledge and understanding of community agencies and resources will be required to ensure participants are served on a holistic level.
Reports to: Program Director
Principal Duties: Workforce Connections Program Assistant
- Manage a caseload of participants and provide counseling and mentoring
- Assess participant competencies, work history, educational attainment, skills, and abilities, and identify challenges to finding employment
- Conduct regular individual meetings with participants for job search, education, job retention, and job readiness
- Create an employment plan for short- and long-term goals and maintain information about area resources and employers
- Create and conduct referrals to community resources, including medical, mental health services, etc.
- Ensure participant accountability and attendance, track and maintain employment retention goals
- Maintain scheduled contact with participants to assess job retention/advancement issues
- Provide information to participants on available training and job opportunities and assist with job applications, resumes, etc.
- Monitor participant progress from referral through retention and advancement efforts
- Prepare and maintain accurate participant files reflecting the entire history of a program participant
- Provide case management to help participants prepare for employment, post-secondary, or military outcomes
- Coordinate individual instruction and improve job readiness scores of students in workshops
- Ensure completion and submission of assignments as scheduled
- Maintain participant assessments in files and participant database
- Collaborate with teams, business customers, and other staff
- Assist participants in establishing and maintaining a healthy support system, living environment, and financial stability
- Ensure in-person contact with participants at least once every 30 days during Phase One of the program and maintain monthly follow-up in Phase Two of the program
- Assist participants through crisis situations and/or arrange for the provision of such assistance
- Attend and participate in IEP meetings as needed
- Perform other duties assigned - scheduled and assigned duties are subject to change based on the needs of the program and the participants we serve
Qualifications & Education & Experience:
- High School Diploma or equivalent
- Bachelor’s degree or 2+ years experience in case management
- Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred
- Preferred experience within the field of human service or non-profit work
- Experience in workforce development or related programs preferred
- Excellent listening skills and oral and written communication
- Skill in interacting effectively with individuals and groups
- Proficient in Google Suite
- Proficient in case management documentation
