Summary:
The main function of a Case Manager is to organize and facilitate patient care and work with healthcare providers to ensure patients receive necessary services. This includes assessing patient needs, evaluating treatment options, and coordinating care.
Job Responsibilities:
- Coordinate and provide care that is safe, timely, and effective.
- Manage case assignments, draft service plans, review case progress, and determine case closure.
- Work closely with clients, their families, and healthcare professionals to ensure comprehensive care.
- Advocate for the patients and their well-being.
Skills:
- Verbal and written communication skills, multi-tasking, customer service skills and interpersonal skills.
- Strong ability to work independently and manage one’s time.
- Strong ability to keep information organized and confidential.
- Previous experience with computer applications, such as Microsoft Word, Excel and PowerPoint.
Qualifications:
- High school diploma or GED required.
- 10+ years’ experience required.
Compensation:
The pay rate range above is the base hourly pay range that Aditi Consulting reasonably expects to pay someone for this position (compensation may vary outside of this range depending on a number of factors, including but not limited to, a candidate’s qualifications, skills, competencies, experience, location and end client requirements).
