Registered Nurse-Special Needs

Work Schedule: The Registered Nurse – Special Needs Coordinator will be on duty for a
minimum of 40 hours per week. Scheduled shifts are normally 0700-1600 CST Monday
thru Friday 9 hours per day (to include a one hour, non-paid, lunch break). Contractors
will not be required to work on weekends and Federal Holidays. The Contractor must, at
all times, maintain an adequate workforce for the uninterrupted performance of all tasks
defined within this PWS when the Government facility is not closed for the above
reasons. Any schedule changes are to be approved by the Functional Requirements
Evaluator Designee and must be submitted to the COR in writing for coordination with
the company.
Duties: The duties for the HCW are as follows:
To provide a Registered Nurse – Special Needs Coordinator (SNC) services which
facilitate the patient’s transition to the appropriate level of care. Work collaboratively
with utilization management, case management and the healthcare team to achieve a
timely, appropriate discharge that meets the patient’s needs and is per local
facility/AF/DoD and national utilization and discharge standards.
- The SNC registered nurse oversees and manages the installation Exceptional Family
Member Program-Medical (EFMP-M) IAW DoD and AF policy and any subsequent
implementing guidance.
- Uses clinical assessment skills and maintains procedures to identify sponsors whose
family members have special medical and educational needs in a timely manner. The
SNC uses a multi-disciplinary and collaborative approach with other key service
providers, such as installation youth and childcare facilities, and officer and enlisted
spouses groups, to ensure effective outreach and identification of special needs. Ensures
family members of the AF personnel or family of the AF member are appropriately “Q
coded” (EFMP-enrolled).
- Ensures all active duty sponsors known to the Air Force Personnel Center, and local
Military Personnel Section (MPS) and/or Commander Support Staff (CSS) where
applicable, as having family members with special needs are identified in AF medical
special needs data management systems. Provides oversight for base-level data entry in
AF-provided data management systems used in the management of EFMP-M.
- Provides training to installation personnel and medical staff; and consultation as needed
to support the implementation of EFMP-M base-wide.
- Integrally involved in the Family Member Relocation Clearance (FMRC) process.
Ensures all FMRC requirements are implemented by all EFMP-M staff. Coordinates the
enrollment process and travel screening for all active duty family members’ and travel
screening upon request to the families of DoD civilian sponsored assignments going to an
overseas base.
- Provides oversight to ensure every AF sponsor with one or more family members with
special needs assigned to the installation has a SN file maintained at the MTF. Ensures Qcoded
sponsors assigned to the installation are contacted annually to determine if there
are unmet needs and to request updates of information as needed.
- Collaborates with the Integrated Delivery System (IDS) and other interagency forums
that provide family medical information and referrals to base and civilian agencies.
Ensures EFMP families are referred to the Airman and Family Readiness Center for
additional community assistance as needed. Maintains a cooperative working relationship
with the base MPS, CSS, and with Air Force Personnel Center (AFPC). Actively supports
the integration of EFMP-M, Exceptional Family Member Program – Family Support
(EFMP-FS) and Exceptional Family Member Program – Assignments (EFMP-A)
services at the installation. Provides information, appropriate contact information, and
coordinates referrals as appropriate.
- Participates in EFMP quarterly case reviews to discuss/assess newly identified
families’, complex, or unmet medical needs. Determine the appropriate resources
necessary for the families.
- Patient specific assessments and clinical plans of action will be located in
AHLTA/MHS GENESIS and the Special Needs record. Meeting minutes will not contain
patient identifying information.
- Coordinates and participates in designated facility meetings.
- Shall consult with medical personnel, legal authorities, and military commanders as
required.
- The contractor shall follow applicable local facility/AF/DoD instructions, policies and
guidelines.
- Must maintain a level of productivity and quality consistent with: complexity of the
assignment; facility policies and guidelines; established principles, ethics and standards
of practice of professional nursing, Health Services Inspection (HSI); and other
applicable DoD and service specific guidance’s and policies. Must also comply with the
Equal Employment Opportunity (EEO) Program, infection control and safety policies and
procedures.
- The contractor shall complete medical record documentation and coding, and
designated tracking logs and data reporting as required by local facility/AF/DoD
instructions, policies and guidance.
- The contractor will complete all required electronic medical record training, facilityspecific
orientation and training programs, and any AF/DoD mandated training.
- The contractor shall ensure the employee is in compliance with preventive, prophylactic
and follow-up procedures, as well as infection control and employee health program
procedures, as established by the MTF. Required preventive, prophylactic and follow-up
procedures will be provided by the Government to contract employees in the military
MTF. The HCW shall reimburse payment for these services. If care is received
elsewhere, the HCW shall provide written verification of treatment.
- Contractor employees shall prepare all documentation to meet or exceed established
MTF standards, to include, but not limited to: timeliness, accuracy, content, and
signature. Contractor employees shall only use MTF and Air Force-approved
abbreviations for documentation in the patient health records and follow the MTF’s Do
Not Use Abbreviation List. (AFI 48-123, Medical Examinations and Standards)
- Health care providers shall complete medical records documentation within 72 business
hours of the encounter with the patient.
- Contractor employees shall use the proper coding of procedures and treatment, and shall
maintain coding accuracy (CPT/E&M/procedure) at the same standard, 90% or higher, as
military and civil service health care providers engaged in comparable work.
- Shall be responsible for creating, maintaining and disposing of only those Government
records which are specified by the 96 MDG.
- Shall possess basic knowledge of and ability to demonstrate leadership skills in order to
provide guidance, counseling, training and professional evaluation.
- Shall possess basic knowledge in management and daily operation of a clinic servicing a
large number of patients with a variety of healthcare needs and illnesses.
- The SNC may be asked to participate in committees, programs or clinical services
directly related to area of clinical responsibility or privileges.
- Shall provide for ongoing education programs to promote, protect and maintain
optimum levels of wellness. Shall provide health counseling to patients and their families,
and develop a plan for health maintenance based on individual needs.
- Shall direct supporting Government employees assigned to them during the performance
of clinical duties. Guides the performance of assigned personnel. Uses team approach
toward staff and patient management.
- Shall perform administrative duties that may include maintaining statistical records of
his or her clinical workload, participating in medical educations programs, and
participating in quality improvement functions and other duties as prescribed by the Flight
Commander/Medical Management Director/Chief, Health Care Integrator.
- Shall attend and participate in meetings, professional staff conferences and other
appropriate professional activities such as, but not limited to the following: quality
improvement meetings, professional staff meetings, commander's staff meetings, flight
meetings, and other meetings required by applicable regulations, MTF guidance, or as
directed by the Flight Commander/Medical Management Director/Chief, Health Care
Integrator or his/her designated representative.
- Shall possess basic knowledge of operating a wide variety of medical equipment/nonmedical
equipment to ensure patient and staff safety, proper equipment maintenance and
accessibility.
- Shall treat patient information, no matter how developed, as privileged information.
Patient information shall not be disclosed or revealed in any way for any use outside the
MTF without prior written permission by the Chief of Hospital Services (SGH).
- Shall respect and maintain the basic rights of patients, demonstrating concern for
personal dignity and human relationships. Providers receiving complaints validated by
the Functional Requirements Evaluator Designee (FRED), COR and SGH shall be
subject to counseling from the contractor’s company and, depending on the nature and
severity of the complaint, separation from performing services under this contract.
- Shall maintain open and professional communication with members of the MTF staff,
MTF customers, and other contractor employees. COR will report valid complaints in
writing to the Contracting Officer (CO) and the contractor’s company for action. Valid
complaints will be considered a performance failure. The contractor shall resolve all
substantiated complaints immediately upon notification of the complaint and no later than
the suspense date provided by the CO or designated representative.
- Shall seek and maintain interpersonal relationships with clients/families and coworkers
which contribute to mission accomplishment.
- Shall possess basic knowledge of various training methods and interpersonal skills in
order to develop a rapport with the patients and co-workers during which instructional
and education information is presented and a supportive, trusting relationship is
established.
- Shall possess ability to acquire and use basic computer skills to enter patient data and
extract patient’s information, from a variety of sources to include MHS GENESIS,
Composite Health Care System (CHCS), Armed Forces Health Longitudinal Technology
Application (AHLTA), Essentris and Integrated Clinical Database (ICDB).
- Mastery of theories, principles, and methodologies underlying psychosocial practice.
- Knowledge and understanding of developmental growth; dynamics of human behavior,
family, and other social systems; and impact of illness and disability on social
functioning.
- Demonstrated abilities in program planning, implementation, and evaluation, and in
conducting individual, family, group, and community assessments.
- Strong capability to work with service members and/or veterans and their families who
are experiencing a wide range of complicated mental, emotional, behavioral, physical,
psychosocial, and environmental problems.
- Knowledge of medical privacy and confidentiality (Health Insurance Portability and
Accountability Act [HIPAA]), and accreditation standards of Accreditation Association
for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC). Working
knowledge of computer applications/software to include Microsoft Office programs, MS
Outlook (e-mail), and internet familiarity is required. The ability to input, extract and
format data from established databases.

- The SNC may need to attend EFMP-M training in order to remain current with DHA
policy and program rules. This may require face-to-face attendance at an out-of-state
location (location TBD) for intensive program updates. Additionally, the SNC may need
to schedule/perform site visits and training for outlying/support EFMP programs that
would require travel to/from those locations in the surrounding areas. Video
teleconferencing is largely unavailable and system requirements don’t allow for
consistent abilities to make this a viable option for training. The Government shall
reimburse the contractor for travel costs in accordance with the travel guidance in the
contract MQS Basic Contract PWS.
Qualifications
Qualifications:
Degree/Education (5.2.4): Must be a graduate of an associates (ADN) or baccalaureate degree (BSN) program in nursing accredited by a national nursing accrediting agency recognized by the US Department of Education.
Certifications in addition to Basic Life Support (5.2.3): The contractor shall hold current American Heart Association or the American Red Cross Healthcare Provider (HCP) or Basic Life Support (BLS) certification. Certification granted based solely on computer-based training does not constitute fulfillment of this requirement.
Experience: A minimum of 2 years full-time experience in care coordination, discharge planning or case management in a medical inpatient or outpatient setting within the last four years; Registered Nurses should have a minimum of 3 years for ADN/2 years for BSN full-time experience in nursing and care coordination.
Must have knowledge and skills to effectively apply Special Needs Coordinator (SNC) functions: a) Assessment: Identification of patients that need special medical and education needs/management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care; c) Facilitation: Care coordination and communication among all involved parties; d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care coordination is received.
Licensure/Registration: Registered Nurses must maintain an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as in any US state/jurisdiction.
