Job Overview:

We are looking for a skilled and compassionate Nurse Care Manager with specialized experience in wound care, catheter care, and geriatrics. The ideal candidate will have a strong background in these areas and a commitment to delivering high-quality patient care.

Key Responsibilities:

- Oversee and manage comprehensive wound care programs, including assessment, treatment, and ongoing monitoring of patients with acute and chronic wounds.

- Provide expert catheter care management, ensuring proper insertion, maintenance, and monitoring to prevent infections and complications.

- Develop and implement individualized care plans for geriatric patients, addressing their unique health needs and promoting their overall well-being.

- Coordinate with healthcare providers, patients, and families to ensure a seamless continuum of care.

- Educate patients and their families on wound care, catheter care, and geriatric health management.

- Maintain accurate and detailed patient records, documenting care provided and any changes in patient conditions.

- Ensure compliance with all healthcare regulations and best practices.

Qualifications:

- Valid Registered Nurse (RN) license.

- Proven experience in wound care, catheter care, and geriatrics.

- Strong clinical skills and knowledge in the specified areas.

- Excellent communication and interpersonal skills.

- Ability to work effectively as part of a multidisciplinary team.

- Compassionate and patient-focused approach to care.

- Previous experience in a care management role is preferred.
