About this Role:

We are a rapidly growing virtual specialty care practice who are passionate about improving access to high quality specialty care. As our patient population grows we’re expanding our clinical operations department to streamline patient care processes, support our healthcare providers, and ensure excellent overall patient outcomes.

We’re seeking a Care Coordinator who’s excited to play a crucial role in ensuring an exceptional patient experience throughout a patient’s healthcare journey. In this role you will serve as a liaison between patients and other members of the patient’s care team as you coordinate appointments, manage referrals, and provide prescription and lab support. You’ll work directly with patients and their families, insurance representatives, pharmacies, laboratories, and physicians to ensure a seamless experience from start to finish.

Who You Are:

Have previous experience as a care coordinator, ideally in a healthcare startup environment.
Excellent communication, time management, planning, organizational, and problem-solving skills.
Willing to work a flexible schedule including evenings, weekends and holidays
Proficient with Slack and G-suite and comfortable learning an intuitive, easy-to-use EHR.
You genuinely care about the well-being of patients and are passionate about being on the front lines of healthcare, changing patient lives.
You can empathize with a patient's concerns and show compassion in your interactions.
You are an adaptable collaborator who enjoys digging in to get to the root cause of an issue, and you’ll go the extra mile to support your team or improve the patient experience.

Key responsibilities of the Patient Care Advocate include:

Act as the primary point of contact for patients, addressing their non-clinical questions and providing assistance to any issue that may arise;
Schedule and coordinate patient appointments;
Perform pre-visit registration and onboarding;
Verify patients' insurance coverage and eligibility for services;
Facilitate referrals for in-person care or other services;
Confirm prescriptions, complete prior authorizations, and coordinate lab results;
Monitor and ensure timely completion of necessary follow-up actions, as directed by the medical team;
Coordinate administrative clinical functions of the practice.

Remote Requirements:

Ability to multitask through multiple programs and systems comfortably in a remote office setting;
Proficient with tech problem solving and set up;
Secure, reliable and dedicated high-speed internet;
Must have a dedicated and private home work space.
