Job Overview
As a dedicated Care Coordinator, you will be pivotal in delivering exceptional patient care through effective coordination services. In this remote role, your primary function involves working closely with a designated group of patients to provide monthly care coordination services, particularly focusing on chronically ill individuals. You will educate patients regarding their conditions and facilitate treatment plans with their healthcare providers, ensuring a holistic approach to their care.
Key Responsibilities
- Conduct monthly Chronic Care Management (CCM) phone calls with patients, healthcare practices, and pharmacies, dedicating 3-4 hours daily.
- Create and modify personalized care plans for each patient in collaboration with supporting healthcare providers.
- Identify and address social determinants of health, ensuring each patient’s needs are met through appropriate resources and referrals.
- Empower patients to take charge of their health and wellness goals within their care plans.
- Maintain patient confidentiality and adhere to all ethical, legal, and regulatory standards consistently.
Required Skills
- Strong communication and collaboration abilities, fostering effective relationships with patients and healthcare colleagues.
- Exceptional active listening and critical thinking skills to respond appropriately to patient inquiries.
- Proficiency in using various EMR/EHR systems, along with Microsoft Office and Google Suite.
- Ability to manage a high volume of calls while meeting established productivity benchmarks.
- Compassionate demeanor with excellent written and verbal communication skills.
Qualifications
- A minimum of 1 year of experience as a Certified or Registered Medical Assistant, with active accreditation.
- Experience in a call center or triage environment is advantageous.
- Familiarity with serving geriatric populations and previous experience in case management or care coordination is a plus.
