Summary
Under the direction and supervision of the ICMS Program Manager, the Program Supervisor will be
responsible for providing leadership, mentoring, and training to our Intensive Case Management
Teams. Oversight of daily operations and day-to-day activities at the Compton office and field-based case management. Program Supervisor’s primary responsibilities will be to conduct daily tracking of Case manager activities, be responsible for day-to-day oversight of field operations and office operations, provide field-based support as needed and monitor follow-through of client linkages and referrals.
Essential Functions
- Provide direct oversight to the Intensive Case Management Services Case Managers
- Maintain an in-depth knowledge of Department of Health Services and Department of MentalHealth policies and procedures and implementing changes as needed
- Implement policies and procedures and ensure the delivery of high quality ICMS services bymonitoring the operations if the ICMS program
- Responsible for coordinating with Department of Health Services, Department of MentalHealth and Brilliant Corners to ensure the seamless transition of homeless clients intopermanent housing
- Document case consultations with ICMS Case Management Staff and inform ProgramManager of findings
- Oversee and provide direction to ensure that case management staff assist clients at every state of the housing stabilization process
- Track data related to DHS reporting, client referrals, case management caseloads, and service-related trends
- Responsible for monitoring client progress and service-related experience
- Cultivate resources and collaborate with community partners that will enhance serviceplanning and delivery
- Identify knowledge gaps and provide appropriate ongoing outreach training to address stafftraining deficits
- Implement and monitor tracking systems related to staff productivity
- Ensure that ICMS case managers receive appropriate trainings
- Available on an on-call basis for emergency situations encountered by ICMS case management staff
- Monitor the crisis response system for all ICMS clients
- Attend regularly scheduled and as-needed coordination and/or team meetings with DHS
- Work closely with property management agency and other supportive service agencies,including case management and mental health providers to ensure clients’ needs are being met
- Maintain appropriate boundaries; and adhere to SSG’s Code of Ethics and HOPICS’ CoreValues
- Represent the Agency in a professional manner at meetings and community events
- Maintain files/records on client services in compliance with HIPAA & CFR42 and otherfunding requirements for auditing purposes
- Regular attendance required
- Bilingual candidates are highly desired
Minimum Qualifications - Knowledge, Skills and Abilities Required
- Bachelor’s degree from an accredited university OR 5 years’ experience working in the socialservice field
- Knowledge of the Los Angeles County homeless services landscape
- California Driver License, Proof of Car Insurance, Reliable transportation,
- Ability to organize and manage multiple priorities,
- Experienced on Microsoft Word and Excel
- If in recovery, a minimum of three (3) years of being drug and alcohol free isMANDATORY
- Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds
- Verification of Employment Eligibility and Background Check
- Ability to communicate effectively, both written and orally
- Ability to work with homeless clients with chronic physical and behavioral impairments
- Holiday or weekend work may be required
- Knowledge of SPA 6 homeless landscape
- Familiar with Homeless Management Information System
Supervisory Responsibilities
- The Program Supervisor will be responsible for a team of field-based case managers
Environmental Conditions (Working Conditions)
- The environment for this position is office and field based
- Must be able to work in an environment with many priorities, busy, and fast paced
- Must be able to problem solve, adapt to changes that are unpredictable
- Must be able to work with highly aggressive clients
- Must be able to work in various shelter environments
- Exposed to highly aggressive clients: must communicate too many sources including courtsand outside community agencies: sometimes noisy, loud and disruptive clients
- Some evenings and weekends are required
Physical Requirements
- While performing this job, the incumbent typically spends time sitting, standing, walking, speaking, thinking, listening, and carrying (20lbs). Must sit at the computer for data entry and reports
- sometimes spend time driving and walking and carrying equipment/files/charts. Assisting clients with moving their personal belongings.
- Speak to staff inside and outside agencies in public meetings and Provider Meetings
- Represent the Agency in a professional and positive manner
- TB test required (Not more than (3) months prior to or (7) days after date of hire and renewal annually thereafter.)
- CPR and First Aid Certification required within 30 days of employment with company
